Directly ask if you could return. A humbling experience. No excuses, no blame. Take full accountability for your actions and consequences.I’m leaning towards asking my old firm for my old job back. I feel like a damn fool but this isn’t what I signed up for.

You've been gone months. Other employees have kept the org afloat without you.
Probably some self-deprecation would be recommended and hopefully you'll get some razzing from them.
Maybe some have already taken "your" office, "your" chair, "your" parking spot, etc. If you go back, let them keep all that.
Some who have taken your workload may want to keep it for the challenge/growth. Mentor them on it; be willing to take their regular work instead.
Your professional growth must be addressed by you if you go back. Your former employer let you walk away after hearing this as a reason and didn't make a satisfactory pitch to retain you, so your growth upon return will be your responsibility. Don't expect the employer to make your work fit your growth goals....I didn’t feel like I was growing...
