how does insurance monitor all of such things?
It is common internet misinformation that insurance will deny claims traced to unpermitted renovations. Read your policy, you will find no discussion of permits.
Let's discuss the following renovation - fire pit in living room and removal of all smoke detectors. Do you think that insurance will pay the claim in case of fire?
The fact that you had to come up with such an absurd example only proves my point. Smoke detectors are required on every level according to my policy, so this would invalidate my policy (and most others I would assume).

No mention is made of permitting anywhere in my policy. No mention is even made of renovations being up to code, or needing to be reported to insurance. No mention is made of plumbing and electrical, other than to tell me that electrical fires or burst/leaking pipes (except due to freezing) are covered losses under my policy.

Your inspector is correct and your realtor (as usual for this career field) is clueless and talking out of their lane.
