Nothing.
Our family stopped doing gifts for adults a decade ago, and it has been nice. Everyone is either retired and financially stable, or gainfully employed. Everyone can buy what they want when they actually want or need it, and we don't have to participate in the social or cultural pressure and stress to engage in mass consumerism. Spending quality time together is the gift.
My personal policy is, if I find something unique that is perfect for someone and that they'd be unlikely to get on their own, then I give it as a gift (typically unwrapped, or wrapped with recycled materials) as soon as possible. You never know whether both parties will make it to the next defined gift-giving occasion. So why wait and deprive them of enjoyment over what could be months of time?
