From what I understand investors can create their own 3 fund portfolios in 401ks (dependent on fund availability) and IRAs?

Yep. Since you mentioned that you have Fidelity funds available in your 401(k), look for the following three:

FSKAX - Total US Stock
FTIHX - Total International Stock
FXNAX - Total Bond
