I tried to break the tie between Alan and Neuro by mocking this up in TT and it didn't even squeak about the excess, even when I made one W2 show 40k D/AA - nothing!
That means it's not a tax filing issue. Ie without a 1099R you are doing nothing wrong. The question about following up with your 401k admin is completely separate. Probably Alan can explain the process (I'm most interested to hear how a pre-tax basis can would even tracked in a Roth)
btw you probably woulda gotten a 1099R code P this year already, since all this was from last year. Seems like they may not detect this w/o you bringing it up to them.
I double-checked just now, on their workday page, they only give me a W-2. They never sent a 1099-R
